目的研究介入治疗后的室间隔缺损(VSD)患儿的认知能力及行为问题,分析两者之间的关联性。方法选择自2010年1月-2012年12月在山东省立医院进行经导管介入封堵治疗的VSD 64例患儿。采用P300事件相关电位来评估VSD患儿的认知能力,另外采用儿童行为问题问卷来评估患儿的情绪和行为问题,分析这两者之间的关联性,并以56位正常对照儿童参与了本研究。结果 1)与对照组相比,VSD患儿在额叶和顶叶部位具有长的潜伏期,额叶和顶叶部位的波幅明显低于对照组;2)VSD的男童存在抑郁、体诉等行为问题,女童存在抑郁、强迫、社会退缩、违纪等行为问题;3)存在抑郁、体诉问题的VSD男童事件相关电位的异常分别表现为顶叶潜伏期延长、波幅降低及额叶潜伏期延长;4)存在抑郁、强迫、社会退缩、违纪问题的VSD女童事件相关电位的异常分别表现为顶叶潜伏期延长和波幅降低、额叶波幅降低、额叶潜伏期延长、顶叶波幅降低。结论与正常儿童相比,介入治疗后的VSD患儿,学龄期的认知能力存在轻度的损害,且有一定程度的情绪和行为问题,VSD患儿的行为问题与异常的事件相关电位有一定的相关性。
Objective To investigate the cognitive and behavioral problems of children with ventricular septal defect (VSD) after interventional therapy and to analyze the relationship between the two. Methods From January 2010 to December 2012, 64 cases of VSD were treated by catheterization and closure in Shandong Provincial Hospital. P300 event-related potentials were used to assess children’s cognitive ability in children with VSD. Children’s behavioral questions questionnaire was also used to evaluate children’s emotional and behavioral problems. The correlation between the two was analyzed and 56 normal control children were involved this research. Results 1) Compared with the control group, children with VSD had long latency in the frontal lobe and parietal lobe, and the amplitude of frontal lobe and parietal lobe was significantly lower than that in the control group. 2) There was depression, Behavioral problems, girls’ depression, coercion, social withdrawal, disciplinary violations and other behavioral problems; 3) Depression, physical prosecution of VSD boys associated with the event-related potential abnormalities were the parietal lobe latency, amplitude and frontal latency; 4) The abnormal events related to the events of the girls with depression, coercion, social withdrawal and violation of discipline were the prolongation and amplitude reduction of the parietal lobe latency, the decrease of the amplitude of the frontal lobe, the prolongation of the frontal lobe latency, and the decrease of the amplitude of the parietal lobe. Conclusion Compared with normal children, children with VSD after interventional therapy have mild cognitive impairment in school age and have some emotional and behavioral problems. The behavioral problems in children with VSD are related to abnormal events A certain degree of relevance.
