目的探讨经阴道超声在“导管环”型早期未破裂型输卵管妊娠与类“导管环”型早孕时厚壁黄体囊肿鉴别诊断中的应用价值。方法回顾性分析36例“导管环”型早期未破裂型输卵管妊娠患者(输卵管妊娠组)和40例类“导管环”型早孕时厚壁黄体囊肿患者(黄体囊肿组)的经阴道超声声像资料,比较2组“导管环”外径大小,血流阻力指数,高于卵巢回声、等于或低于卵巢回声比例,周边见卵泡比例,环状及半环状血流比例,点状及条状血流比例。结果输卵管妊娠组“导管环”外径大小((2.46±1.45)cm)、高于卵巢回声比例88.9%(32/36)、等于或低于卵巢回声比例11.1%(4/36)、周边见卵泡比例2.8%(1/36)、环状及半环状血流比例11.1%(4/36)、点状及条状血流比例88.9%(32/36)与黄体囊肿组((3.12±1.08)cm、10.0%(4/40)、90.0%(36/40)、95.0%(38/40)、85.0%(34/40)、15.0%(6/40))比较差异有统计学意义(P<0.05),血流阻力指数(0.45±0.09)与黄体囊肿组(0.43±0.11)比较差异无统计学意义(P>0.05)。结论经阴道超声可从“导管环”外径大小、回声及彩色血流分布上对“导管环”型早期未破裂型输卵管妊娠与类“导管环”型早孕时厚壁黄体囊肿进行鉴别诊断。
Objective To investigate the value of transvaginal ultrasound in the differential diagnosis of thick-wall luteal cyst with “Catheter ring” type early unruptured tubal pregnancy and type “Catheter ring” type early pregnancy. Methods The clinical data of 36 patients with tubal ring-type early unruptured tubal pregnancy (tubal pregnancy) and 40 patients with thick-wall luteal cyst (corpus luteum cyst group) were retrospectively analyzed. Vaginal ultrasound imaging data, compared 2 groups “catheter ring ” diameter size, blood flow resistance index, higher than the ovarian echo, equal to or lower than the ovarian echo ratio, peripheral see follicular ratio, annular and semi-circular blood flow Proportion, punctate and strip ratio of blood flow. Results The diameter of the “catheter ring” in the tubal pregnancy group was (2.46 ± 1.45) cm, higher than the ovarian echo ratio of 88.9% (32/36), equal to or lower than the ovarian echo ratio of 11.1% (4/36) Peripheral follicles were seen in 2.8% (1/36), annular and semi-circular blood flow was 11.1% (4/36), point-like and strip-shaped blood flow ratio of 88.9% (32/36) and luteal cyst group ( 3.12 ± 1.08) cm, 10.0% (4/40), 90.0% (36/40), 95.0% (38/40), 85.0% (34/40), 15.0% (6/40) (P <0.05). There was no significant difference between the blood flow resistance index (0.45 ± 0.09) and corpus luteum cyst (0.43 ± 0.11) (P> 0.05). Conclusion Transvaginal ultrasonography can be used to evaluate the effect of transvaginal catheterization on the early “undisturbed tubal pregnancy” and “catheter ring” type early pregnancy Luteal cyst for differential diagnosis.
