目的探讨采用重组人脑利钠肤(rhBNP)治疗急性心肌梗死后心力衰竭的临床效果及患者循环内分泌的变化。方法 72例急性心肌梗死后心力衰竭患者,按照患者意愿分为对照组和观察组,各36例。对照组行常规治疗,观察组采用rhBNP治疗,对两组患者的治疗效果及其循环内分泌激素水平变化情况进行观察分析。结果经治疗,观察组患者左室射血分数(LVEF)、左室舒张末期内径(LVEDd)均明显高于对照组,血浆BNP水平明显低于对照组,差异均具有统计学意义(P<0.05);观察组患者在治疗结束时醛固酮(ALD),血管紧张素Ⅱ(AngⅡ)及肾素活性(PRA)水平均明显低于对照组,差异具有统计学意义(P<0.05)。结论采用rhBNP治疗急性心肌梗死后心力衰竭,能够有效抑制心室重构,改善心功能,并对于循环内分泌系统功能异常具有十分显著的抑制作用,是一种有效的心力衰竭治疗药物。
Objective To investigate the clinical effects of recombinant human brain sodium and rhodium (rhBNP) on heart failure after acute myocardial infarction and the changes of circulating endocrine in patients. Methods A total of 72 patients with heart failure after acute myocardial infarction were divided into control group and observation group according to patients’ wishes, each of which was 36 cases. The control group received routine treatment, and the observation group was treated with rhBNP. The therapeutic effect and the changes of circulating endocrine hormone levels in the two groups were observed and analyzed. Results After treatment, left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDd) in the observation group were significantly higher than those in the control group, and plasma BNP levels were significantly lower than those in the control group (P <0.05 ). The levels of aldosterone, angiotensin Ⅱ and renin (PRA) in the observation group were significantly lower than those in the control group at the end of treatment, the difference was statistically significant (P <0.05). Conclusion rhBNP treatment of heart failure after acute myocardial infarction can effectively inhibit ventricular remodeling, improve cardiac function, and has a very significant inhibitory effect on circulatory endocrine system dysfunction, which is an effective therapeutic drug for heart failure.
