背景:目前尚缺乏满意的治疗脑梗死的手段。亚低温治疗脑梗死虽然有效,但副作用较大。局部亚低温对脑梗死的治疗可能有较好的作用。目的:观察局部亚低温对大鼠缺血脑组织的保护作用,进一步探讨其作用机制。设计:以实验动物为研究对象,随机对照的基础研究。单位:一所大学医院的神经内科研究所。材料:实验于2000-04/2002-01在哈尔滨医科大学第一临床医学院神经内科动物实验室完成,动物为雄性Wistar大鼠50只,体质量(250±25)g,清洁级。干预:50只大鼠中随机取10只,分为正常组和假手术组,每组5只。余40只随机分为常温脑缺血组和局部亚低温脑缺血组,各20只。制作大鼠大脑中动脉脑缺血模型,进行局部亚低温处理。主要观察指标:各组大鼠脑梗死灶体积、神经功能和血清神经元特异性烯醇化酶的影响。结果:大鼠栓塞48h时亚低温组和常温组大鼠脑梗死体积分别为(128.95±13.42)和(84.90±11.36)mm3,神经功能评分分别为1.60±0.24和0.95±0.17,血清神经元特异性烯醇化酶浓度分别为(13.55±4.07)和(9.19±3.42)μg/L,差异均有显著性意义。结论:局部亚低温治疗对缺血脑神经元具有保护作用。
Background: Currently there is a lack of satisfactory means of treating cerebral infarction. Mild hypothermia treatment of cerebral infarction although effective, but the side effects. Local mild hypothermia in the treatment of cerebral infarction may have a better effect. Objective: To observe the protective effects of local mild hypothermia on ischemic brain tissue in rats and to explore its mechanism. Design: The experimental animal as a research object, a randomized controlled basic research. Unit: a University Hospital Institute of Neurology. MATERIALS: Experiments were performed at the Department of Neurology, First Clinical College of Harbin Medical University from April 2000 to January 2002 with 50 male Wistar rats weighing 250 ± 25 g, clean grade. Intervention: 10 rats were randomly selected from 10 rats, divided into normal group and sham operation group, with 5 in each group. More than 40 patients were randomly divided into normothermic ischemic group and local mild hypothermia cerebral ischemia group, 20 in each. Rat middle cerebral artery occlusion model was made and subjected to local mild hypothermia. MAIN OUTCOME MEASURES: Effects of cerebral infarct size, neurological function and serum neuron specific enolase of rats in each group. Results: The volume of cerebral infarction was (128.95 ± 13.42) and (84.90 ± 11.36) mm3 in the mild hypothermia group and the normal temperature group at 48 hours after embolization, respectively. The neurological scores were 1.60 ± 0.24 and 0.95 ± 0.17, respectively. Serum neuron specific The concentrations of enolase were (13.55 ± 4.07) and (9.19 ± 3.42) μg / L, respectively, with significant differences. Conclusion: Local mild hypothermia has a protective effect on ischemic cerebral neurons.
