目的 探讨乳房早发育( 简称乳早) 患儿的发病机理及寻找易行的反映雌激素水平的方法加以监护。方法 对50 例乳早患儿采用Shorr 染色技术行尿脱落细胞检查及放射免疫双抗法检测血清促性腺激素和雌二醇水平。结果 乳早患儿尿底层细胞构成比显著低于正常对照组( P< 0-01),而中层及表层细胞构成比均显著高于正常对照组( P均< 0-01) ;乳早患儿乳房发育均属Ⅱ级或Ⅲ级,Ⅲ级患儿尿底层细胞构成比显著低于Ⅱ级患儿( P<0-01),而表层细胞构成比显著高于Ⅱ级患儿( P < 0-01) ,中层细胞构成比两级患儿之间无显著差异( P >0-05) ;乳早患儿与正常对照组血清促性腺激素、雌二醇比较均无显著差异( P均> 0-05) ;乳早患儿血清雌二醇与尿底层细胞构成比呈弱度负相关(r= - 0-49, P < 0-01),与尿表层细胞构成比呈弱度正相关(r= 0-59 , P <0-01) ,与尿中层细胞构成比相关性不明显(r= 0-24,P> 0-05) 。结论 乳早患儿尿道上皮受到雌激素影响;尿表层细胞构成比能灵敏反映体内雌激素水平,可作为观察病情演变指标。
Objective To investigate the pathogenesis of early breast development (early maternal age) and to find ways to monitor estrogen levels easily. Methods Serum gonadotropin and estradiol levels were measured by Shorr staining in 50 cases of early lactation infants with urine exfoliated cells and radioimmunoassay. Results The percentage of urinary basal cells in early infants was significantly lower than that in the normal control group (P <0-01), while the ratio of middle and superficial layers was significantly higher than that in the normal control group (all P <0-01) (P <0-01), while the constituent ratio of superficial cells was significantly higher than that of grade Ⅱ children (P < There was no significant difference between the two groups (P> 0-05). There was no significant difference in serum gonadotropin and estradiol in early-onset infants > 0-05). The composition ratio of serum estradiol to urine in the early stage of infant milk samples was negatively correlated with the degree of weakness (r = - 0-49, P <0-01) Correlation (r = 0-59, P <0-01) showed no significant correlation with urinary mesenchymal cells (r = 0-24, P> 0-05). Conclusion Early urinary tract epithelial cells in early lactation are affected by estrogen. The composition of urothelial cells reflects the estrogen level sensitively, which can be used as an index to observe the disease.
