目的:探讨机械通气在急性左心衰竭治疗中的应用时机选择。方法:将32例采用早期机械通气的急性左心衰患者作为治疗组,28例未采用早期机械通气的患者作为对照组,观察两组患者机械通气后0.5h、1h、3h的呼吸频率(RR)、动脉血氧分压(PaO2)、心率(HR)、平均动脉压(MAP)等变化情况,及患者住院病死率。结果:两组患者机械通气后0.5、1、3h的RR、PaO2、HR、MAP较通气前有明显改善(P<0.05);治疗组中30例缓解,2例死亡,对照组中20例缓解,8例死亡,两组住院病死率差异有统计学意义(P<0.05)。结论:在救治急性左心衰患者时尽早应用机械通气能有效改善低氧血症、提高抢救成功率。
Objective: To explore the timing of mechanical ventilation in the treatment of acute left heart failure. Methods: Thirty-two patients with acute left heart failure treated with early mechanical ventilation were selected as the treatment group and 28 patients without early mechanical ventilation were used as the control group. The respiratory rate of the two groups after 0.5h, 1h, 3h ), PaO2, heart rate (HR), mean arterial pressure (MAP), and hospital mortality. Results: The RR, PaO2, HR and MAP at 0.5, 1, 3 h after mechanical ventilation in both groups were significantly improved compared with that before ventilation (P <0.05). In the treatment group, 30 patients were relieved, 2 patients died, and 20 patients in the control group were relieved , 8 died, the difference between the two groups in hospital mortality was statistically significant (P <0.05). Conclusion: As soon as possible in the treatment of acute left heart failure patients with mechanical ventilation can effectively improve the hypoxemia and improve the success rate of rescue.
