目的通过对本站制备的不同血浆制品中有效凝血因子的促凝活性,纤维蛋白原和血浆总蛋白含量的测定,为深入掌握血浆产品的质量、准确解答临床相关血浆输注问题提供基础数据支持,对指导临床合理用血并提高疗效提供帮助。方法随机抽取本站制备的新鲜冰冻血浆、普通冰冻血浆(去冷沉淀冰冻血浆31份,其他36份)、亚甲蓝病毒灭活冰冻血浆,共128份,分别用一期法检测FⅡ、FⅤ、FⅦ、FⅧ、FⅨ、FⅩ、FⅪ、FⅫ的促凝活性,凝血酶法检测纤维蛋白原和双缩脲法检测总蛋白含量,并进行统计学分析。结果血浆总蛋白各组间比较,差异无统计学意义(P>0.05)。其他检测项目组间比较:CRP与FP比较:FⅪ、FⅫ之间P值均>0.05,无统计学意义,其他项均有差异;CRP与MB-FP比较:Fib、FⅤ、FⅦ、FⅫ之间P值均>0.05,无统计学意义,其他项均有差异;CRP与FFP比较:Fib、FⅤ、FⅧ之间P<0.05有意义,其他项均无差异;FP与MB-FP比较:Fib、FⅡ、FⅦ、FⅪ之间P<0.05有统计学意义,其他项均无差异;FP与FFP比较:Fib、FⅡ、FⅫ之间P值均>0.05,无意义,其他项均有差异;MB-FP与FFP比较:FⅦ之间P>0.05,无统计学意义,其他项均有差异。结论不同血浆制品有效成分各不相同,临床上不能等同使用。经过灭活的MB-FP纤维蛋白原含量显著降低,与CRP水平相当,其他因子也有不同程度的降低;CRP因提取了冷沉淀与FP比较,纤维蛋白原含量显著降低,其他因子也各有差别,应作为一种成分与FP区别使用。
OBJECTIVE: To provide the basic data for further understanding of the quality of plasma products and accurately answering clinical related problems of plasma coagulation by measuring the procoagulant activity, the content of fibrinogen and the total plasma protein of the effective coagulation factors in different plasma products prepared in this site , To guide clinical rational use of blood and improve the efficacy of help. Methods A total of 128 fresh frozen plasma, normal frozen plasma (31 cryoprecipitate frozen plasma, 36 others) and methylene blue virus-inactivated frozen plasma were randomly selected. A total of 128 FⅡ, FⅤ , FⅦ, FⅧ, FⅨ, FⅩ, FXI, FⅫ. The content of total protein in fibrinogen and biuret assay was detected by thrombin assay and statistical analysis was performed. Results There was no significant difference in plasma total protein between groups (P> 0.05). Other test items were compared between groups: CRP and FP: F Ⅺ, F Ⅻ between P values were> 0.05, no statistically significant, other items are different; CRP and MB-FP: Fib, F Ⅴ, F Ⅶ, F Ⅻ between P values were all higher than 0.05, there was no significant difference between the other items; CRP and FFP: Fib, FV, FⅧ between P <0.05 meaningful, other items were no difference; FP and MB-FP: Fib, FⅡ, FⅦ, FXI between the two groups were statistically significant (P <0.05). There was no difference in other items between FP and FFP. P values of Fib, FⅡ and FⅫ were all> 0.05, FP and FFP: F Ⅶ between P> 0.05, no statistical significance, the other items are different. Conclusion Different plasma products have different active ingredients and can not be used clinically. After inactivation of MB-FP fibrinogen content was significantly reduced, with CRP levels, other factors also have varying degrees of reduction; CRP extracted cold precipitation compared with FP, fibrinogen levels were significantly reduced, and other factors are also different , Should be used as a component and FP distinction.
