目的研究血脂组分及其比值与2型糖尿病(T2DM)合并冠心病(CHD)发病风险的关系,及其与ATP结合盒转运体-A1(ABCA1)基因R219K多态性的关联,探讨T2DM合并CHD的风险因子。方法用PCR-RFLP方法分析正常人群(Con组)、T2DM患者(T2DM组)和T2DM合并CHD患者(T2DM+CHD组)ABCA1基因R219K多态性,对比各组血脂组分水平以及不同组分的比值变化,分析ABCA1基因R219K多态性和血脂变化与T2DM合并CHD发病的关系。结果 (1)Con组、T2DM组和T2DM+CHD组的等位基因K频率分别为42.4%、39.7%和31.7%,三组KK基因型频率分别为19.3%、15.4%和5.6%,其中T2DM+CHD组等位基因K和基因型KK频率均显著低于其他两组(P<0.05)。(2)基因型KK者血浆HDL-C水平显著高于RK和RR者(P<0.05),而TG水平显著低于RK和RR者(P<0.05)。(3)T2DM+CHD组LDL-C/HDL-C比值显著升高(P<0.05),但T2DM组与Con组间无统计学差异。结论血脂组分比值可真实地反映T2DM合并CHD的风险。三种基因型中,KK纯合子能有效降低LDL-C/HDL-C比值,从而降低T2DM合并CHD的风险。
Objective To investigate the relationship between lipid components and its ratio and the risk of type 2 diabetes mellitus (T2DM) complicated with coronary heart disease (CHD) and its association with the R219K polymorphism of ATP binding cassette transporter-A1 (ABCA1) gene, CHD risk factors. Methods The R219K polymorphism of ABCA1 gene in normal subjects (Con group), T2DM patients (T2DM group) and T2DM patients with CHD (T2DM + CHD group) was analyzed by PCR-RFLP. The levels of serum lipids, Ratio changes, analysis ABCA1 gene R219K polymorphism and changes in blood lipid and T2DM associated with CHD incidence. Results The frequencies of allele K in Con group, T2DM group and T2DM + CHD group were 42.4%, 39.7% and 31.7%, respectively. The frequency of KK genotype in the three groups was 19.3%, 15.4% and 5.6% + CHD group, allele K and genotype KK frequency were significantly lower than the other two groups (P <0.05). (2) Plasma HDL-C levels of genotype KK were significantly higher than those of RK and RR (P <0.05), while TG levels were significantly lower than those of RK and RR (P <0.05). (3) The LDL-C / HDL-C ratio in T2DM + CHD group was significantly increased (P <0.05), but there was no significant difference between T2DM group and Con group. Conclusion The ratio of serum lipids can truly reflect the risk of T2DM complicated with CHD. Among the three genotypes, KK homozygotes can effectively reduce the LDL-C / HDL-C ratio, thereby reducing the risk of T2DM complicated with CHD.
