目的探讨螺旋CT胰腺增强扫描的最佳时间窗。方法选取福建医科大学附属宁德市医院2015年1月—2016年10月收治的胰腺无病变患者100例,其中40例患者接受螺旋CT同层增强扫描,60例患者接受螺旋CT多期增强扫描,评价动脉期、胰腺期螺旋CT对胰周血管的显示率及显像效果评分。结果 40例患者行螺旋CT同层增强扫描后发现,胰周主动脉、胰周门静脉、胰腺实质、肝脏实质强化到达峰值的平均时间分别为30 s、45 s、40s、60 s,其中,胰周主动脉在注射对比剂20 s后,CT值已达200 HU。60例患者行螺旋CT多期增强扫描后发现,动脉期胰周大动脉显示率达100%,显像效果评分为2.93~2.95分,胰周其他动脉显示率为78%~98%,显像效果评分为0.85~2.63分。胰腺期的胰周大静脉显示率达100%,其显像效果评分为2.80~2.88分,胰周其他静脉显示率为90%~93%,显像效果评分为1.08~1.20分。结论当对比剂总量为120 ml、注射速率为5 ml/s时,螺旋CT增强扫描胰腺动脉期、胰腺期、肝脏期最佳时间窗分别为20 s、40 s、60 s。
Objective To explore the best time window of spiral CT pancreas enhanced scanning. Methods 100 cases of pancreas-lesion-free patients admitted to Affiliated Ningde Hospital of Fujian Medical University from January 2015 to October 2016 were selected. Forty patients underwent spiral CT scans, 60 patients underwent multi-phase helical CT scan, Evaluation of arterial phase, pancreatic spiral CT of the perivascular vascular rate and imaging scoring. Results The average time of peak enhancement in the peri-pancreatic aorta, peripancreatic portal vein, pancreatic parenchyma and liver was 30 s, 45 s, 40 s and 60 s, respectively, in 40 patients undergoing helical CT scans. Among them, The aorta in the injection of contrast agent after 20 s, CT value has reached 200 HU. 60 patients underwent spiral CT multi-phase enhanced scan showed that the rate of peri-arteries in the arterial phase was 100%, the scoring effect was 2.93-2.95, and the rate of other peripancreatic arteries was 78% -98% The score is 0.85 ~ 2.63. Pancreatic pancreas perineal venous display rate of 100%, the imaging score was 2.80 ~ 2.88 points, other peripancreatic peripheral venous display rate of 90% to 93%, the imaging effect score of 1.08 to 1.20 points. Conclusion When the total amount of contrast medium is 120 ml and the injection rate is 5 ml / s, the optimal time window of pancreas phase, pancreas phase and liver phase in spiral CT scan is 20 s, 40 s and 60 s, respectively.
