目的:研究不同植骨材料和超声骨刀联合治疗第一磨牙根分叉病变骨下袋的治疗效果。方法:选择第一磨牙根分叉病变骨下袋病例102例,随机分为4组:A组实施翻瓣术,应用超声骨刀,B、C组在此基础上分别植入羟基磷灰石骨粉和Bio-Oss骨粉,D组实施翻瓣术,植入Bio-Oss骨粉,在基线及术后1、6、12月检测牙周临床指标和根分叉区骨密度。结果:四组病例术后临床指标均有所改善,与基线比较有显著性差异(P<0.05),但组间比较无显著性差异(P>0.05)。C组在骨密度增加方面较A、B、D组明显,而B、D组又较A组增加明显,均有统计学意义(P<0.05)。结论:超声骨刀联合Bio-oss骨粉植入治疗根分叉病变能获得更多的牙周再生和牙槽骨的重建。
OBJECTIVE: To study the therapeutic effect of different bone graft materials combined with ultrasonic osteotome in the treatment of subglotic pocket of root bifurcation lesions of first molars. Methods: A total of 102 cases of primary genicular root bifurcation lesions were selected and randomly divided into 4 groups. In group A, flap flap was applied. Ultrasound osteotome and B and C groups were implanted with hydroxyapatite Bone meal and Bio-Oss bone meal. Group D was treated by flap surgery. Bio-Oss bone meal was implanted. Periodontal clinical indexes and root bifurcation bone mineral density (BMD) were measured at baseline and at 1, 6 and 12 months after operation. Results: The clinical indexes of the four groups improved after operation, there was significant difference compared with baseline (P <0.05), but there was no significant difference between the two groups (P> 0.05). Group C showed more significant increase in bone mineral density than Group A, B and D, while Group B and D increased significantly compared with Group A (all P <0.05). CONCLUSIONS: Ultrasound osteotome combined with Bio-oss bone meal implantation can achieve more periodontal regeneration and alveolar bone remodeling for root bifurcation lesions.
