目的：为探讨咽旁隙肿瘤的外科治疗方法，进一步减少术后并发症，提高手术成功率。方法：本文回顾性总结１９８１～１９９４年以来４５例咽旁隙肿瘤的外科治疗，其中良性肿瘤４０例，恶性肿瘤５例，根据患者的实际情况，采用了４种手术入路。结果：肿瘤全切４４例，术后颅神经并发症１１例（神经源性肿瘤８例，腮腺肿瘤３例），其中８例于术后６个月之内恢复，随诊中，良性肿瘤术后复发３例，恶性肿瘤术后３年生存率４０％（２／５）。结论：结果表明，咽旁隙肿瘤术后并发症主要为颅神经功能障碍。仔细解剖其毗邻关系和剥离肿瘤是手术成功的关键，大多数患者可望恢复，并对咽旁隙手术的进路，肿瘤的病理类型，手术要点及术后并发症进行总结和讨论。
Objective: To investigate the surgical treatment of parapharyngeal space tumors, to further reduce postoperative complications and improve the success rate of surgery. Methods: This article retrospectively reviewed the surgical treatment of 45 parapharyngeal space tumors from 1981 to 1994, including 40 benign tumors and 5 malignant tumors. According to the actual situation of patients, four surgical approaches were used. RESULTS: All the tumors were resected in 44 cases. There were 11 postoperative cranial nerve complications (8 cases of neurogenic tumors and 3 cases of parotid gland tumors). Among them, 8 cases recovered within 6 months after operation. Follow-up examination was performed on benign tumors. After recurrence in 3 cases, the 3-year survival rate of malignant tumors was 40% (2/5). Conclusion: The results show that the complications of postoperative pharyngeal space tumors are mainly cranial nerve dysfunction. Careful anatomy of its adjacent relationship and dissection of the tumor is the key to successful surgery. Most patients are expected to recover, and the approach to parapharyngeal surgery, the pathological type of the tumor, the major points of operation, and postoperative complications are summarized and discussed.
