[目的]分析良性及中间型骨纤维类肿瘤的影像细节征象,以提高诊断准确率。[方法]回顾性分析31例经手术病理证实或活检穿刺证实的的良性(纤维性骨皮质缺损6例、非骨化性纤维瘤11例)及中间型(良性纤维组织细胞瘤8例、骨韧带样纤维瘤6例)骨纤维类肿瘤的影像学表现特征。[结果]良性纤维类骨肿瘤表现为膨胀性骨质破坏,边界清楚,有硬化边(13/17),无骨膜反应及骨旁软组织肿块;中间型纤维类骨肿瘤骨质破坏区无硬化边(9/14),可有骨旁软组织肿块(5/14)及少量骨膜反应(3/14);良性及中间型无骨化纤维类肿瘤可以有残留的骨嵴但没有骨化,可有少量钙化(2/31)。[结论]良性及中间型纤维类骨肿瘤影像学征象能间接反映其病理组织学特点,多种影像学方法的联合应用能够提高鉴别诊断能力。
[Objective] To analyze the imaging detail signs of benign and intermediate type of fibrous bone tumors in order to improve the diagnostic accuracy. [Methods] Totally 31 benign cases (6 cases of fibrous cortical defect, 11 cases of non-ossifying fibroma) and 8 cases of benign fibrous histiocytoma (benign fibrous histiocytoma) confirmed by pathology and biopsy were retrospectively analyzed. [Results] The benign fibrous bone tumors presented with expansive bone destruction, clear boundary, hardened edge (13/17), no periosteal reaction and periosteal soft tissue mass. The fibrosis type of intermediate fibrous bone mass had no sclerosis (9/14), there may be para-soft tissue mass (5/14) and a small amount of periosteal reaction (3/14); benign and intermediate non-ossifying fibroids may have residual ridges without ossification, may have Small amount of calcification (2/31). [Conclusion] The imaging signs of benign and intermediate fibrous tumors can indirectly reflect the histopathological features. The combination of multiple imaging methods can improve the differential diagnosis ability.
