目的:观察醒脑静注射液联合高压氧对重度颅脑损伤患者神经功能恢复的影响。方法:2014年10月~2016年9月我院神经外科收治的68例重症颅脑损伤患者,所有患者均常规治疗基础上使用高压氧治疗,其中34例在以上治疗基础上另外加用醒脑静注射液。观察两组患者治疗后神经功能的恢复情况。结果:治疗一个月后,观察组血清ET-1、S100B和NPY水平分别为(50.58±8.79ng/L、0.41±0.10μg/L、74.5±11.4ng/L),均明显低于对照组(63.82±9.94ng/L、0.60±0.15μg/L、93.7±12.0ng/L)。治疗3个月后,观察组MMSE、NIHSS评分和记忆商值分别为(27.26±2.19分、15.89±1.49分、96.81±4.15),均明显优于对照组(23.85±2.05分、20.14±1.53分、85.99±3.96)。治疗3个月后,对照组有2例患者死亡,观察组无死亡患者,观察组预后良好(IV+V)比例为50.00%,明显高于对照组26.47%。(4)对照组患者治疗期间发生20例(58.82%)肺部感染,明显高于观察组11例(32.35%)。结论:在高压氧治疗重度颅脑损伤患者基础上,醒脑静注射液可提高患者神经功能恢复程度,并减少感染的发生。
Objective: To observe the effects of xingnaojing injection combined with hyperbaric oxygen on the recovery of neurological function in patients with severe craniocerebral injury. Methods: From October 2014 to September 2016, 68 patients with severe craniocerebral injury admitted to Department of Neurosurgery in our hospital were enrolled. All patients were treated with hyperbaric oxygen on the basis of routine treatment. Among them, 34 patients were additionally treated with Xingnao Static injection. The recovery of neurological function after treatment in both groups was observed. Results: After a month of treatment, the levels of ET-1, S100B and NPY in observation group were (50.58 ± 8.79ng / L, 0.41 ± 0.10μg / L, 74.5 ± 11.4ng / L), which were significantly lower than those in control group 63.82 ± 9.94 ng / L, 0.60 ± 0.15 μg / L, 93.7 ± 12.0 ng / L). After 3 months of treatment, MMSE, NIHSS score and memory quotient in the observation group were significantly (27.26 ± 2.19, 15.89 ± 1.49, 96.81 ± 4.15, respectively), which were significantly better than those in the control group (23.85 ± 2.05, 20.14 ± 1.53 , 85.99 ± 3.96). Three months after treatment, 2 patients died in the control group, and no death patients in the observation group. The prognosis of the observation group was good (50.00%), which was significantly higher than that of the control group (26.47%). (4) In the control group, 20 cases (58.82%) of pulmonary infection occurred in the treatment group, which was significantly higher than that in the observation group (11 cases, 32.35%). Conclusion: Based on hyperbaric oxygen therapy in patients with severe craniocerebral injury, Xingnaojing Injection can improve the recovery of neurological function and reduce the incidence of infection.
