患者,女,42岁,因患鼻窦炎口服鼻炎宁冲剂1包,5分钟后出现胸闷,喉部阻塞感,头晕、恶心、大汗。查体:神志淡漠,呼吸26次/min,面色苍白,四肢湿冷,全身散布粉红色粟粒样大小皮疹。血压0,脉搏细数,心率130次/min,心音微弱。考虑为过敏性休克。即予肌注肾上腺素1mg,地塞米松5mg,吸氧,予5%GS500ml+多巴胺60mg,5%GS500ml+地塞米松10mg,分两个通道静滴,50%GS20ml+10%葡萄糖酸钙10ml静注等抗休克处理。密切观察患者表现及生命特征变化,2小时后血压升至90/60mmHg,患者自觉症状减轻,皮疹消褪。继续用药,6小时后患者症状消失,精神可,呼吸21次/
