经鼻高流量湿化氧疗用于气管切开患者的基础研究及改进

目的:评估经鼻高流量湿化氧疗(HFNC)及人工气道连接管加延长管用于气管切开患者时不同流量、潮气量对进入至隆突部位气体的温度、湿度及氧浓度的影响,寻找一种更加有效的用于气管切开患者的HFNC方式。方法:建立HFNC模型,测定不同流量及潮气量组合下气管内温度、湿度及氧浓度的变化,观察潮气量及流量对气管内温度、湿度及氧浓度的影响;测定人工气道连接管加延长管后不同流量及潮气量组合下气管内温度、湿度及氧浓度的变化,观察延长管对气管内温度、湿度及氧浓度的影响。结果:同等潮气量前提下,流量越大,到达隆突部位气体的温度越高,湿度越大,氧浓度越高;同等流量前提下,潮气量越大,到达隆突部位的温度越低,湿度越小,氧浓度越低。人工气道连接管加延长管后同等流量及潮气量前提下,可提高到达隆突部位气体的温度、湿度及氧浓度。结论:HFNC用于气管切开患者时进入隆突部位的气体温度、湿度及氧浓度受潮气量、流量的影响;人工气道连接管加延长管后可提高HFNC效率。“,”Objective:To evaluate the effects of different flow rate and tidal volume on the temperature, humidity and oxygen concentration of the gas entering the carina site during high flow nasal cannula oxygen therapy (HFNC) for tracheotomy patients.And evaluated the effects of the extension tube.To evaluate the effects of the extension tube added to artificial airway connecting tube on the temperature, humidity and oxygen concentration of the gas entering the carina site and to find a more effective HFNC method for tracheotomy patients.Methods:A model was constructed to allow HFNC.The changes of temperature, humidity and oxygen concentration in trachea were measured under different flow and tidal volume combinations.The effects of tidal volume and flow rate on temperature, humidity and oxygen concentration in trachea were observed.The changes of temperature, humidity and oxygen concentration in trachea under different flow rate and tidal volume combinations were measured after the extension tube added to artificial airway connecting tube.The effects of extention tube on temperature, humidity and oxygen concentration in trachea were observed.Results:Under the same tidal volume, the higher the flow rate was, the higher the temperature, humidity and oxygen concentration of the gas entering the carina site were.Under the same flow rate, the higher the tidal volume was, the lower the temperature, humidity and the oxygen concentration of the gas entering the carina site were.When the flow rate was set at 20 L/min and 40 L/min, the temperature, humidity and oxygen concentration of the gas entering the carina site could be increased after the artificial airway connecting tube was added with an extension tube.Conclusions:When HFNC is used for tracheotomy patients, the temperature, humidity and oxygen concentration of the gas entering the carina site are affected by the tidal volume and flow.The efficiency of HFNC can be improved by adding an extension tube to the artificial airway connecting tube.
