幼年特发性关节炎(JIA)患儿存在免疫紊乱,易发生各种感染。本文探讨了口服匹多莫德对预防JIA患儿感染及对其免疫功能的影响。选取JIA患儿共60例作为研究对象,按治疗方案分为一般治疗组(n=30)和匹多莫德组(n=30)。匹多莫德组在治疗原发病的基础上加用匹多莫德口服。观察治疗前后12个月内JIA患儿感染情况,包括感染次数、病程、部位,外周血T淋巴细胞亚群变化(CD3+,CD4+,CD8+,CD4+/CD8+等),IgG、IgM、IgA、C3和C4水平,NK细胞(CD16+56+)数量及功能变化以及匹多莫德口服液的副作用。另取30例正常儿童作为治疗前的健康对照组。比较匹多莫德组和一般治疗组治疗前后的感染情况以及外周血免疫功能变化。结果显示,接受匹多莫德治疗的患儿控制感染的总有效率(90%)较一般治疗组(36.7%)明显增高(P<0.01)。两组治疗后12个月相比,匹多莫德组CD3+、CD4+、CD4+/CD8+,IgG、IgM、IgA,自然杀伤细胞活性、C3均明显高于一般治疗组,CD8+则明显低于一般治疗组(P<0.05)。提示匹多莫德能够促进JIA患儿免疫异常的恢复,有效预防感染发生。
Infantile idiopathic arthritis (JIA) children with immune disorders, prone to various infections. This article explored the effect of oral pidotimod on the prevention of JIA infection in children and their immune function. A total of 60 children with JIA were enrolled in this study. Patients were divided into general treatment group (n = 30) and pidotimod group (n = 30). Pidotimod group in the treatment of primary disease based on the use of Pidotomide oral. The infection status of JIA children within 12 months before and after treatment including infection frequency, course of disease, location, changes of T lymphocyte subsets (CD3 +, CD4 +, CD8 +, CD4 + / CD8 +), IgG, IgM, IgA, C3 and C4 levels, the number and function of NK cells (CD16 + 56 +) and the side effects of pidotimod oral solution. Another 30 normal children as a healthy control group before treatment. The changes of infection status before and after treatment and changes of immune function in peripheral blood in Pidotimod group and general treatment group were compared. The results showed that the total effective rate of infection control (90%) in children treated with Pidotimod was significantly higher than that in the general treatment group (36.7%) (P <0.01). CD3 +, CD4 +, CD4 + / CD8 +, IgG, IgM, IgA, natural killer cell activity and C3 in the pidotimod group were significantly higher than those in the general treatment group at 12 months after treatment Group (P <0.05). Tip Pidotimod can promote the recovery of immune abnormalities in children with JIA, effectively prevent the infection.
