目的:评估Semont管石解脱法治疗后半规管良性阵发性位置性眩晕(benign paroxysmal positional vertigo of the pos-terior semicircular canal,P-BPPV)的短期和长期疗效,探讨处理P-BPPV的有效模式。方法:回顾分析2007年4月～2008年6月间对应用红外线视频眼动记录分析而确诊为P-BPPV36例患者资料,对所有患者行Semont管石解脱法治疗,并采用门诊和家庭自我治疗相结合的模式。在治疗结束后l周及3个月复查和评价疗效。结果:28例(77.8%)患者症状于治疗后l周内消失,5例症状减轻,3例无效。总有效率为91.7%(33/36)。3个月复查,24例治愈,3例有效,2例无效,7例复发(19.4%),总有效率75%(27/36)。结论:用Semont管石解脱法,并采用门诊和家庭自我治疗相结合的模式治疗P-BPPV具有良好的短期和长期疗效,能有效地减轻患者症状,提高生活质量,值得在临床推广使用。 Objective: To evaluate the short-term and long-term curative effect of benign paroxysmal positional vertigo of the pos-terior semicircular canal (P-BPPV) after treatment with Semont’s tubal lysis and to explore the effective mode of treatment for P-BPPV. Methods: The data of 36 patients with P-BPPV confirmed by eye movement recording and analysis using infrared video from April 2007 to June 2008 were retrospectively analyzed. All patients were treated with Semont tube stone exfoliation method and outpatient and family self-treatment A combination of modes. At the end of treatment l weeks and 3 months review and evaluation of efficacy. Results: The symptoms of 28 patients (77.8%) disappeared within 1 week after treatment, 5 patients had relieved symptoms and 3 patients were ineffective. The total effective rate was 91.7% (33/36). Three months of follow-up, 24 were cured, 3 were effective, 2 were ineffective, 7 were relapsed (19.4%) and the total effective rate was 75% (27/36). Conclusion: The treatment of P-BPPV with the combination of out-patient clinic and family self-treatment has good short-term and long-term curative effect with Semont tube stone release method. It can effectively alleviate the symptoms of patients and improve the quality of life. It is worth to be used clinically.
