目的探讨闭合复位屈肘外翻位固定结合外固定架治疗肱骨髁上骨折的疗效。方法 40例小儿肱骨髁上骨折患者平分为两组,治疗组与对照组各20例,两组首先都采用外固定架治疗,治疗组在此基础上加用闭合复位30度屈肘外翻位石膏固定治疗。结果按照关节功能评价指标,经过治疗后,治疗组优良率为85.0%;对照组优良率为70.0%,治疗组的术后关节功能优良率明显好于对照组,差异有显著性(P<0.05)。治疗组的临床愈合时间平均为(18.5±6.2)d,对照组平均为(25.9±4.3)d,治疗组的治疗组的临床愈合时间明显少于对照组(P<0.05)。随访6个月,治疗组肘内翻发生率为35.0%,肘内翻发生率为5.0%,治疗组的肘内翻发生率明显少于对照组(P<0.05)。结论闭合复位屈肘外翻位固定结合外固定架治疗肱骨髁上骨折能有效提高术后关节功能优良率,加快临床愈合时间,降低肘内翻发生率,值得推广应用。
Objective To investigate the curative effect of closed reduction elbow flexion combined with external fixator in the treatment of supracondylar humerus fractures. Methods Forty pediatric patients with supracondylar humerus fractures were divided into two groups, the treatment group and the control group, 20 cases. The two groups were treated with external fixator. The treatment group was treated with closed reduction of 30 degrees elbow valgus Gypsum fixed treatment. Results According to the evaluation of joint function, the excellent and good rate was 85.0% in the treatment group and 70.0% in the control group. The excellent and good joint function rate of the treatment group was significantly better than that of the control group (P <0.05 ). The mean clinical healing time was (18.5 ± 6.2) days in the treatment group and (25.9 ± 4.3) days in the control group. The clinical healing time of the treatment group was significantly less than that of the control group (P <0.05). After 6 months of follow-up, the incidence of cubitus varus in the treatment group was 35.0% and the incidence of cubitus varus was 5.0%. The incidence of cubitus varus in the treatment group was significantly less than that in the control group (P <0.05). Conclusion Closed reduction elbow valgus combined with external fixator in the treatment of supracondylar humerus fractures can effectively improve the rate of postoperative joint function, speed up the clinical healing time and reduce the incidence of cubitus varus, which is worth popularizing and applying.
