目的:观察四磨汤口服液加非营养性吸吮(NNS)对早产儿胃食管反流(GER)的影响。方法:将47例需经鼻饲管喂养(INGF)的健康早产适于胎龄儿,用同一种配方乳喂养。根据是否辅以四磨汤口服液加非营养性吸吮随机分为非营养性吸吮组(NNS)和单纯鼻饲管喂养组(N-NNS)。记录入液量、奶量、热卡及肠道营养达418.4kJ/(kg.d)的时间,记录喂养相关情况;同时进行食管24hpH值监测,记录以下指标:24h内总反流次数;反流指数(RI);反流持续时间>5分次数;pH<4总时间;最长酸反流时间。结果:四磨汤组24h内反流次数明显少于对照组分别为6(0~24)次,13(6~28)次,P<0.05;pH<4的总时间和反流指数与对照组比较明显下降;肠道营养达418.4kJ/(kg.d)的时间比对照组明显缩短分别为(10.24±3.83)天,(15.75±4.55)天,P<0.05。结论:早产儿鼻饲管喂养期间给四磨汤口服液加非营养性吸吮是一种安全简单的辅助喂养方法,可有效的减少胃食管反流次数,对胃肠动力发育有促进作用,有助于早产儿生后肠道营养的建立。
Objective: To observe the effects of Siu Tang decoction plus non-nutritive sucking (NNS) on gastroesophageal reflux (GER) in preterm infants. METHODS: Forty-seven healthy preterm infants fed with nasal feeding tube (INGF) were suitable for gestational age and fed with the same formula. According to whether it is supplemented by Siu Tang oral liquid plus non-nutritional sucking randomly divided into non-nutritional sucking group (NNS) and nasal feeding group (N-NNS). Record the amount of fluid, milk, heat card and intestinal nutrition up to 418.4kJ / (kg.d) of time, record the feeding related situation; at the same time 24hpH value of esophageal monitoring, record the following indicators: total reflux within 24h; Flow Index (RI); Reflux Duration> 5 Minutes; pH <4 Total Time; Longest Acid Reflux Time. Results: Four milliliters of the group within 24 hours of reflux was significantly less than the control group were 6 (0 ~ 24) times, 13 (6 ~ 28) times, P <0.05; pH <4 total time and reflux index and control (P <0.05). The time of intestinal nutrition up to 418.4kJ / (kg.d) was significantly shorter than that of the control group (10.24 ± 3.83 days, P <0.05), (15.75 ± 4.55) days respectively. CONCLUSIONS: It is a safe and easy way to feed non-nutritive sucking to Sirao Tang oral solution during nasogastric feeding in premature infants, which can effectively reduce the frequency of gastroesophageal reflux and promote the development of gastrointestinal motility.
