患儿女,14岁。因发热、关节痛、指(趾)端溃疡2个月,并心悸、面部红斑出现10天,抽搐1次,以系统性红斑狼疮(SLE)收入院。体检:体温37.8℃,脉搏140次,血压82/60mmHg。精神差,不能坐立、行走。手掌跖有斑片状红斑,指(趾)端发凉伴溃疡,指关节屈曲,甲床发绀,颧部蝶形红斑,骶尾部有3×4cm深在渍疡。心界略大,心率140次/分,第Ⅰ心音低钝,呈奔马律,未闻扩杂音及心包摩擦音。双肺散
