目的探讨血流动力学指标检测在大面积脑梗死中的诊断价值。方法选择大面积脑梗死患者35例为观察组,30例健康志愿者为对照组,所有研究对象均接受CVDI检查,观察组患者均接受颅脑CT检查。观察并比较两组患者下列CDVI指标:Vmean、Vmax、Vmin、Qmean、Zc及DR,比较CT及CDVI对大面积脑梗死诊断的阳性率。结果①与对照组比较观察组患者病灶部位及镜像部位Vmean、Vmax、Vmin及Qmean均显著降低,Zc和DR均显著升高(P﹤0.05)。与观察组患者镜像侧相比,观察组患者病灶部位Vmean、Vmax及Vmin均显著降低(P﹤0.05)。②CDVI共显示27例患者存在血流动力学异常,阳性率为77.14%,CDVI检查阳性率显著高于CT(51.43%),两者间差异有统计学意义(t=5.0400,P=0.0248)。结论 CDVI能够清晰的显示大面积脑梗死患者血流动力学特点,具有较高的诊断阳性率。
Objective To investigate the diagnostic value of hemodynamic indexes in large area cerebral infarction. Methods 35 cases of large area cerebral infarction were selected as the observation group and 30 healthy volunteers as the control group. All the subjects were examined by CVDI, and the observation group received brain CT examination. The following CDVI indicators were observed and compared between the two groups: Vmean, Vmax, Vmin, Qmean, Zc and DR. The positive rates of CT and CDVI in diagnosing large area cerebral infarction were compared. Results ① Compared with the control group, Vmean, Vmax, Vmin and Qmean in the lesion and the mirror site of the observation group were significantly decreased, and the Zc and DR were significantly increased (P <0.05). Compared with the mirror side of the observation group, the Vmean, Vmax and Vmin of the lesion in the observation group were significantly decreased (P <0.05). ② CDVI showed a total of 27 patients with hemodynamic abnormalities, the positive rate was 77.14%, CDVI positive rate was significantly higher than CT (51.43%), the difference was statistically significant (t = 5.0400, P = 0.0248). Conclusion CDVI can clearly show the hemodynamic characteristics of patients with large area cerebral infarction, with a high diagnostic positive rate.
