[目的]健脾益气活血方对脾虚型血小板聚集缺陷症糖盏蛋白(glycocalicin,GC)表达的影响,探讨其治疗该病的可能机制。[方法]127例脾虚型血小板聚集缺陷症患者随机分为中药组与西药组,中药组82例用健脾益气活血方治疗,西药组45例用安络血治疗,2组均治疗2个月;观察2组治疗后脾虚症状改善、止血疗效及血小板聚集恢复率;采用酶联免疫吸附法检测2组治疗前后及52例健康人(正常对照组)的血浆GC含量。[结果]中药组脾虚症状改善总有效率87.81%、西药组为64.44%,中药组止血总有效率为89.02%、西药组为37.78%,中药组血小板聚集功能恢复率64.63%、西药组仅4.44%;2组比较均差异有统计学意义(P<0.01)。2组患者治疗前GC含量均高于正常对照组(P<0.05);中药组治疗后患者的GC含量下降明显优于西药组(P<0.05),与正常对照组比较差异无统计学意义(P>0.05)。[结论]健脾益气活血方降低脾虚型血小板聚集缺陷症患者高于正常的GC含量,抑制GPⅠbα过度水解可能是其止血和恢复血小板聚集功能的疗效机制之一。
[Objective] The purpose of this study was to investigate the effect of “Jianpi Yiqi Huoxue Decoction” on the expression of glycocalicin (GC) in spleen deficiency syndrome with spleen deficiency syndrome and to explore the possible mechanism of its treatment. [Method] 127 cases of spleen deficiency type platelet aggregation deficiency patients were randomly divided into traditional Chinese medicine group and western medicine group. 82 cases of traditional Chinese medicine group were treated by invigorating the spleen and invigorating qi and activating blood. 45 cases of western medicine group were treated by the anti-collateral blood, Month. The improvement of spleen-deficiency symptoms, hemostatic efficacy and platelet aggregation recovery rate were observed in two groups. The plasma GC levels of two groups before and after treatment and 52 healthy people (normal control group) were detected by enzyme-linked immunosorbent assay. [Results] The total effective rate of TCM in the treatment of spleen deficiency was 87.81% in the traditional Chinese medicine group and 64.44% in the western medicine group. The total effective rate in the traditional Chinese medicine group was 89.02%, that in the western medicine group was 37.78%, that in the traditional Chinese medicine group was 64.63% %; The difference between the two groups was statistically significant (P <0.01). The content of GC in the two groups before treatment was significantly higher than that in the normal control group (P <0.05). The GC content in the Chinese medicine group was significantly lower than that in the western medicine group (P <0.05), and there was no significant difference compared with the normal control group P> 0.05). [Conclusion] Jianpi Yiqi Huoxue Recipe can reduce the GC content of patients with spleen deficiency platelet aggregation deficiency and inhibit the over-hydrolysis of GPⅠbα may be one of the curative mechanisms of hemostasis and platelet aggregation recovery.
