目的探讨单个前哨淋巴结阳性的早期乳腺癌患者,实施保腋窝的改良根治手术的临床应用价值。方法将2013年5月-2015年9月期间于清远市人民医院就诊的122例伴有单个前哨淋巴结阳性的早期乳腺癌患者作为本研究对象,随机分为观察组和对照组,每组61例患者,其中观察组给予保留患者腋窝的改良早期乳腺癌根治手术,对照组患者给予传统的乳腺癌改良根治手术,术后比较两组患者的手术时间、所有患者的上臂皮肤感觉、术中出血量以及术后患者的上肢运动情况。结果实施保留腋窝的改良乳腺癌根治术患者的手术时间以及术中的出血量均明显低于实施传统的乳腺癌改良根治术组的患者,同时上臂局部皮肤感觉异常发生率为9.8%(6/61)明显低于实施传统的乳腺癌改良根治术组的患者45.9%(28/61),此外实施保留腋窝的改良乳腺癌根治术患者的患侧上肢运动功能障碍发生率为11.5%(7/61),明显低于实施传统的乳腺癌改良根治术组的患者发生率的47.5%(29/61),差异有统计学意义(P<0.05)。结论对于单个前哨淋巴结阳性的早期乳腺癌患者实施保腋窝的改良乳腺癌根治手术,临床应用效果好,技术相对比较成熟。
Objective To investigate the clinical value of modified radical mastectomy for protecting axillary lymph nodes in single breast cancer patients with positive SLNs. The incidence of local skin sensory abnormalities in the upper arm was 9.8% (6 / 61) was significantly lower than 45.9% (28/61) of patients treated with modified radical mastectomy, and the incidence of upper limb motor dysfunction was 11.5% in modified radical mastectomy patients with preserved armpits 61), which was significantly lower than 47.5% (29/61) of patients treated with modified radical mastectomy. The difference was statistically significant (P <0.05). Conclusions The single breast cancer patients with positive sentinel node positive breast cancer patients with modified axillary surgery, clinical application of good effect, the technology is relatively mature.
