目的 应用显微外科技术施行带蒂吻合血管的空肠移植食管重建治疗小儿良性食管狭窄及闭锁。 方法 距蔡氏韧带 15cm处截取一段空肠 ,解剖出第二直支血管备吻合用 ,结扎第三直支、第四直支为空肠的血管蒂 ,第二直支动脉与颈横动脉吻合后再行消化道重建。 结果 吻合动、静脉各 10对全部成活 ,未发生食管吻合瘘及狭窄 ,出院前均恢复正常饮食。 结论 该手术成功率高 ,并发症少 ,痛苦小 ,使大而复杂的手术小型化 ,一期愈合 ,是治疗小儿良性食管狭窄及闭锁的有效方法。
Objective To apply microsurgical techniques to perform pediatric esophageal stricture reconstruction and esophagectomy for pediatric benign esophageal strictures. Methods A section of jejunum was intercepted at 15cm from the ligament of Chua and dissected to prepare the second straight branch for anastomosis. The third straight branch was ligated and the fourth straight branch was the pedicle of the jejunum. The second branch artery was anastomosed with the transverse cervical artery Gastrointestinal reconstruction. The results of anastomosis, the veins of all 10 survived, esophageal fistula and stenosis did not occur before discharge were restored to normal diet. Conclusion The operation has high success rate, less complications and less pain, which makes the operation of large and complex surgery small and primary healed. It is an effective method to treat benign esophageal stenosis and atresia in children.
