目的:研究血清白细胞(WBC)、C-反应蛋白(CRP)、白细胞介素-18(IL-18)和降钙素原(PCT)在儿童上呼吸道感染诊治中的应用。方法:将192例上呼吸道感染的患儿分为细菌感染组94例、病毒感染组88例,并选择同期体检健康的儿童83例作为对照组,对其血清PCT、CRP、WBC、IL-18指标水平进行测定与比较。结果:细菌感染组患儿血清中PCT、CRP、WBC、IL-18指标水平明显高于病毒感染组及对照组,差异有统计学意义(P<0.05);而病毒感染组患儿血清PCT、CRP、WBC、IL-18与对照组差异无统计学意义(P>0.05)。细菌感染组的血清PCT、WBC阳性率明显高于病毒感染组,差异有统计学意义(P<0.05);细菌感染组的血清CRP阳性率高于病毒感染组,但差异无统计学意义(P>0.05)。结论:PCT、IL-18、CRP、WBC对儿童上呼吸道感染的早期诊断和对症治疗具有重要的指导意义。
Objective: To study the application of serum leucocyte (WBC), C-reactive protein (CRP), interleukin-18 (IL-18) and procalcitonin (PCT) in diagnosis and treatment of upper respiratory tract infection in children. Methods: A total of 192 cases of upper respiratory tract infection were divided into bacterial infection group (94 cases) and virus infection group (88 cases). 83 healthy children in the same period were selected as control group. Serum PCT, CRP, WBC, IL- Indicator levels were measured and compared. Results: The serum levels of PCT, CRP, WBC and IL-18 in bacterial infection group were significantly higher than those in viral infection group and control group (P <0.05). However, the serum PCT, CRP, WBC, IL-18 and the control group showed no significant difference (P> 0.05). The positive rate of serum PCT and WBC in bacterial infection group was significantly higher than that in virus infection group, the difference was statistically significant (P <0.05). The positive rate of serum CRP in bacterial infection group was higher than that in virus infection group, but the difference was not statistically significant (P > 0.05). Conclusion: PCT, IL-18, CRP and WBC are important for the early diagnosis and symptomatic treatment of childhood upper respiratory tract infection.
