血型o的肾脏移植给血型A或B者之后,所发生的免疫性溶血性贫血已有报告。新近为两例病人(血型分别为A型和AB型)用了血型O同一尸体的肾脏进行了肾移植。过去认为,直接抗球蛋白试验,(DAT)和溶血反应归于B排斥,后来又认为DAT阳性是由于A排斥。但是,第二个病人不同于第一个病人,却没有任何溶血反应。两个病人都用了孢子环免疫抑制剂。报告者认为,第二例缺乏溶血的原因是由于血浆中有特殊的血型物质以及患者的ABO亚型所引起的防御作用。
Immune hemolytic anemia has been reported since the kidney of blood group o has been transplanted to blood group A or B patients. Recently, two patients (blood type A and AB, respectively) were given a kidney transplant with the same cadaver of blood type O. In the past that the direct antiglobulin test, (DAT) and hemolysis due to B rejection, and later that the positive DAT is due to A rejection. However, the second patient differed from the first patient in that it did not have any hemolysis. Both patients used sporocyclic immunosuppressants. The report concludes that the second reason for the lack of hemolysis is the defensive effect caused by the presence of specific blood group substances in the plasma and the ABO subtype of the patient.
