目的 :探讨广视野镜倒像装置系统在玻璃体视网膜显微手术治疗复杂视网膜脱离中应用的价值。方法 :将广视野镜倒像装置系统应用于 4 6例复杂视网膜脱离手术 ,2 7例在气态下行眼内半导体激光光凝 ,其中 5例在气体联合重水下完成光凝 ;1 9例在气态下直视观察行巩膜外冷疑 ,术毕 31例玻璃体腔填充 1 6 % C3F8,1 5例填充硅油。结果 :全部病例顺利完成玻璃体切除及气态下眼内操作 ,随访 6个月至 1 5个月 (平均 1 0 .3个月 ) ,治愈率 89.1 % (4 1 / 4 6 ) ,盲目率从术前 84 .8% (39/ 4 6 )下降到术后 6月 32 .6 % (1 5 / 4 6 ) (P<0 .0 1 )。结论 :广视野镜倒像装置系统为现代玻璃体视网膜显微手术提供了一个广角视野 ,它的应用使许多高精度操作能够顺利完成 ,提高了治愈率 ,减少了盲目率 ,是手术不可缺少的重要工具
OBJECTIVE: To evaluate the value of wide-field inverted camera system in the treatment of complex retinal detachment by vitreoretinal microsurgery. Methods: 46 cases of complicated retinal detachment were treated with wide-field inverted camera system, 27 cases underwent intraocular laser photocoagulation in the gas phase, of which 5 cases achieved photocoagulation under gas combined with heavy water. Under direct observation sclera outside doubt, surgery completed 31 cases of vitreous cavity filled with 16% C3F8, 15 cases filled with silicone oil. Results: All patients underwent vitrectomy and intravitreal operation successfully. The follow-up ranged from 6 months to 15 months (mean, 1.03 months). The cure rate was 89.1% (4 1/46) The former 84.8% (39/46) decreased to 32.6% (15/4 6) after operation (P <0.01). CONCLUSIONS: The wide field of view inverted phantom system provides a wide-angle view of modern vitreoretinal microsurgery, and its use enables many high-precision operations to be performed successfully, increasing cure rates and reducing blindness, making surgery an indispensable and important tool
