目的:研究骨桥蛋白(osteopontin,OPN)在乳腺癌各分子亚型中的表达及其临床意义。方法:收集2000年1月至2003年12月第二军医大学长海医院的99例乳腺癌组织标本,免疫组织化学法检测ER(estrogen receptor)、PR(progesteronereceptor)和HER-2(human epidermal growth factor receptor-2)的表达,并据此将乳腺癌患者分为luminal A型、luminal B型、HER-2过表达型和基底细胞样型。免疫组织化学法测定各分子亚型乳腺癌组织中OPN的表达情况,并分析OPN表达与乳腺癌患者临床病理特征的关系。结果:根据ER、PR和HER-2的不同表达情况,99例乳腺癌患者中luminal A型45例、luminal B型27例、HER-2过表达型14例、基底细胞样型13例。OPN在luminal A型、luminal B型、HER-2过表达型和基底细胞样型中表达率分别为22.2%、29.6%、71.4%和61.5%,HER-2过表达型和基底细胞样型乳腺癌组织中OPN表达率均高于luminal A型和luminal B型(P<0.01)。OPN低表达的乳腺癌患者生存率高于OPN高表达者,两者差异有统计学意义(P<0.01)。结论:OPN在基底细胞样型和HER-2过表达型乳腺癌组织中高表达,其表达可作为乳腺癌预后的评估指标之一。
Objective: To study the expression of osteopontin (OPN) in various subtypes of breast cancer and its clinical significance. Methods: Totally 99 specimens of breast cancer from Changhai Hospital of the Second Military Medical University from January 2000 to December 2003 were collected for immunohistochemical detection of estrogen receptor (PR), progesterone receptor (PR) and human epidermal growth factor receptor-2) expression, and accordingly breast cancer patients were divided into luminal A type, luminal B type, HER-2 overexpression and basal cell type. Immunohistochemistry was used to determine the expression of OPN in breast cancer tissues and the relationship between the expression of OPN and the clinicopathological features of breast cancer patients. Results: According to the different expressions of ER, PR and HER-2, there were 45 luminal A cases, 27 luminal B cases, 14 HER-2 overexpression cases and 13 basal cell-like cases in 99 cases of breast cancer. The expression rates of OPN in luminal A type, luminal B type, HER-2 overexpression and basal cell type were 22.2%, 29.6%, 71.4% and 61.5% respectively. The survival rate of breast cancer patients with low expression of OPN was higher than those with high OPN expression, the difference was statistically significant (P <0.01). CONCLUSIONS: OPN is overexpressed in basal cell-like and HER-2 overexpression breast cancers and its expression may be used as a prognostic indicator of breast cancer.
