目的:研究脑室周围-脑室内出血(PVH-IVH)早产儿血小板参数和凝血功能的变化及其临床意义。方法:对35例PVH-IVH早产儿(PVH-IVH组)入院24 h内行活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、纤维蛋白原(Fbg)、D-二聚体(D-D)含量及血小板参数检测。选取同期未发生PVH-IVH的早产儿40例作为对照组。结果:与对照组相比,PVH-IVH组APTT、PT、TT、D-D明显增高(P<0.01),血小板计数(PLT)、血小板比积(PCT)和Fbg明显下降(P<0.01);两组平均血小板体积(MPV)和血小板分布宽度(PDW)比较差异无统计学意义(P>0.05)。结论:PVH-IVH早产儿存在凝血功能障碍及血小板参数异常,可能参与了PVH-IVH发病机制,要重视对早产儿凝血状态的监测,应做好早期干预。
Objective: To study the changes of platelet parameters and coagulation function in premature infants with periventricular-intraventricular hemorrhage (PVH-IVH) and its clinical significance. Methods: Thirty-five patients with PVH-IVH premature infants (PVH-IVH group) underwent active partial thromboplastin time (PTT), prothrombin time (PT), fibrinogen (Fbg ), D-dimer (DD) content and platelet parameter detection. 40 cases of premature infants without PVH-IVH during the same period were selected as the control group. Results: Compared with the control group, APTT, PT, TT and DD in PVH-IVH group were significantly increased (P <0.01), and PLT, PCT and Fbg in PVH- There was no significant difference in mean platelet volume (MPV) and platelet distribution width (PDW) between the two groups (P> 0.05). Conclusion: There are coagulation disorders and abnormal platelet parameters in PVH-IVH premature infants, which may be involved in the pathogenesis of PVH-IVH. Early monitoring of clotting status should be emphasized and early intervention should be done.
