[目的]探讨应用超声心动图和血浆B-型钠尿肽(B-type natriuretic peptide,BNP)水平诊断高血压心功能受损。[方法]选择门诊及住院高血压患者150例,将其中心脏超声示左室肥厚、血浆BNP水平≥100 pg/ml者70例归为A组;左室肥厚、血浆BNP﹤100 pg/ml者80例归为B组;另选健康检查50人作对照(C组)。空腹抽血测血浆BNP浓度,心脏彩超检测左室厚度及左室舒张功能。[结果]A、B两组超声心动图显示左室肥厚及左室舒张功能的测值无显著差异,但A、B组显著高于C组(P﹤0.01)。3组间的血浆BNP水平差异显著。[结论超声心动图结合血浆BNP水平能准确判断高血压患者心脏功能受损情况。
[Objective] To explore the application of echocardiography and plasma B-type natriuretic peptide (BNP) in the diagnosis of impaired cardiac function in patients with hypertension. [Methods] A total of 150 outpatients and inpatients with hypertension were enrolled in this study. Seventy patients with left ventricular hypertrophy and ≥100 pg / ml plasma BNP level were classified as group A. Left ventricular hypertrophy, plasma BNP <100 pg / ml 80 cases were classified as group B; another 50 healthy people as control (group C). Plasma BNP concentration was measured by fasting blood and left ventricular thickness and left ventricular diastolic function were measured by echocardiography. [Results] The results of echocardiography in group A and group B showed no significant difference in left ventricular hypertrophy and left ventricular diastolic function, but the values in group A and group B were significantly higher than those in group C (P <0.01). Plasma BNP levels differed significantly among the three groups. [Conclusion Echocardiography combined with plasma BNP level can accurately determine the cardiac dysfunction in hypertensive patients.
