目的 探讨标准型CD4 4(CD4 4S)基因蛋白在非小细胞肺癌 (NSCLC)中各组织学类型的表达 ,并探讨与淋巴结转移的关系。方法 采用微波免疫组化染色方法 (LSAB)检测CD4 4s在 78例NSCLS中的阳性表达率。结果 NSCLC总阳性表达率为 74 4 % (5 /7) ,鳞癌、腺癌、大细胞癌和腺鳞癌的阳性率分别是 6 3 2 % (2 4 /38)、88 9% (2 4 /2 7)、83 3% (5 /6 )和 71 4 % (5 /7) ,鳞癌与腺癌、大细胞癌之间有非常显著差异 (P <0 0 1)。高、中、低和未分化组的阳性率分别是 6 8 4 % (13/19)、75 8% (2 5 /33)、76 2 % (16 /2 1)和 80 0 % (4/5 ) ,高分化组和未分化组之间有显著性差异 (P <0 0 5 ) ,其它各组之间无显著性差异 (P >0 0 5 )。有转移组和无转移组的阳性率分别为4 8 1% (17/2 7)和 88 2 % (45 /5 1) ,其中强阳性率分别是 2 5 9% (7/2 7)和 74 5 % (38/5 1) ,不论是阳性率还是强阳性率 ,二组之间均有非常显著差异 (P <0 0 1)。结论 CD4 4s阳性表达率高、阳性强度强的病人转移率低 ,CD4 4s可作为区分NSCLC组织学类型及淋巴结转移潜能的辅助指标
Objective To investigate the expression of standard CD4 (CD4 4S) protein in various histological types of non-small cell lung cancer (NSCLC) and to explore its relationship with lymph node metastasis. Methods The positive rate of CD4 4s expression in 78 cases of NSCLS was detected by microwave immunohistochemical staining (LSAB). Results The positive rate of NSCLC was 74 4% (5/7) in squamous cell carcinoma, adenocarcinoma, large cell carcinoma and adenosquamous carcinoma respectively. The positive rates of NSCLC were 63.2% (2/38) and 88 9% There was a significant difference between squamous cell carcinoma, adenocarcinoma and large cell carcinoma (P <0.01). The positive rates of high, middle, low and undifferentiated groups were 68.4% (13/19), 75.8% (25/33), 76.2% (16/2) and 80% (4 / 5). There was a significant difference between well-differentiated and undifferentiated groups (P <0.05). There was no significant difference between other groups (P> 0.05). The positive rates of metastasis and no metastasis were 481% (17/2 7) and 88 2% (45/5 1), respectively. The positive rates were 259% (7/2 7) and 74 5% (38/5 1), both positive rate and positive rate, there was a significant difference between the two groups (P <0.01). Conclusion The positive rate of CD4 4s is high and the positive rate is low. CD4 4s can be used as an auxiliary index to differentiate the histological type and lymph node metastasis potential of NSCLC
