截肢是世界外科史中最早的手术了。外科医生从来不欢喜这种手术,并认为截肢是一种破坏性手术。我们现今的观点把截肢看成是一种建设性手术,其目的是建立一个新的运动器官。我举英国的一些统计数字来说明我们的问题。在英格兰和威尔士,每年有5000名新的截肢病人(苏格兰未统计在内),其中有3700名因周围血管疾病而截肢,而且80%是60岁以上的老年人,其余1300名因外伤(大多为交通事故)、肿痛、感染、先天畸形而截肢。你们可以想象,我国截肢者的病因主要是周围血管疾病和坏疽,每个病人如果想长寿,就有潜在的可能成为双侧截肢者。我们的统计教字表明:初次截肢的病人中,10%在一年以后死亡,20%在两年以后死亡;3~5年后仍活着的病人绝大多数成了双侧截肢者。
Amputation is the earliest surgery in the history of surgery in the world. Surgeons never rejoiced with this procedure and considered amputation as a destructive procedure. Our current view regards amputation as a constructive operation whose purpose is to establish a new moving organ. I use some statistics from the United Kingdom to illustrate our problems. In England and Wales, there are 5,000 new amputees each year (Scotland does not count), of whom 3,700 are amputated due to peripheral vascular disease, 80% are older than 60, and the remaining 1300 are due to trauma (mostly For motor traffic accidents, swelling, pain, congenital malformations and amputations. As you can imagine, the causes of amputees in China are mainly peripheral vascular diseases and gangrene. If each patient wants to live longer, there is a potential for bilateral amputees. Our statistical teachings show that 10% of the patients who were first amputated died after one year, 20% died after two years, and the vast majority of patients who survived 3 to 5 years later became bilateral amputees.
