目的介绍关节镜下治疗儿童胫骨棘骨折的手术方法及临床效果。方法2001年2月～2003年7月对6例儿童胫骨棘骨折在关节镜下复位,利用导线在关节外搭接后将固定线引入,横穿前交叉韧带基底固定骨折块。结果随访6～20个月,所有骨折均在术后4周愈合,无骨折移位。术后3个月关节活动范围均与健侧相同,无关节不稳,前抽屉实验及Lachmann征阴性。结论关节镜下治疗儿童胫骨棘骨折有助于早期诊治合并伤,利用导线在关节外搭接后将固定线引入操作简便易行、复位准确、固定可靠、不需特殊器械、创伤小、并发症少、恢复快等优点,是治疗儿童胫骨棘骨折的首选方法。
Objective To introduce the surgical methods and clinical results of arthroscopic treatment of tibial spine fractures in children. Methods From February 2001 to July 2003, 6 cases of children with tibial spine fractures were treated with arthroscopic reduction. The fixation wires were inserted through the extra-articular wires and fixed through the anterior cruciate ligament. Results All patients were followed up for 6 to 20 months. All the fractures healed 4 weeks after the operation without fracture displacement. The range of motion of the joint after 3 months was the same as that of the contralateral side, without instability of the joint, the front drawer experiment and negative Lachmann’s sign. Conclusion Arthroscopic treatment of tibial spine fractures in children is helpful for the early diagnosis and treatment of comorbid injuries. It is easy and practicable to introduce the fixation wire after lapped by wires outside the joint, and it has the advantages of accurate reset, reliable fixation, no special instruments, less trauma and complications Less, faster recovery, etc., is the preferred method of treatment of children with tibial spine fractures.
