目的了解60Co与深部X线对于颌骨的损害效应以深入研究颌骨放射性骨坏死(ORNJ)的发病机理。方法采用两种放射源按各种不同剂量分割条件对128只豚鼠右侧颌骨照射,并于放疗前后一定时期拔牙,定期进行颌骨局部X线摄片和组织病理学检查。结果深部X线照射豚鼠无论是颌骨损害程度还是全身反应远较60Co为严重,照射后1月内成骨细胞抑制,破骨细胞活动亢进,因而出现骨质吸收;2~4月后骨细胞数目减少,核固缩,空虚陷窝增多,牙周膜内小血管闭塞。放疗后3、5月拔牙共有7例(14.6%)发生了ORNJ。结论相同剂量的深部X线对颌骨组织的损害程度大大超过60Co;拔牙等颌骨创伤因素对ORNJ的发生确有诱发作用
Objective To investigate the damaging effect of 60Co and deep X-ray on the jaw to investigate the pathogenesis of osteoradionecrosis of the jaw (ORNJ). Methods The right jaw of 128 guinea pigs was irradiated by two kinds of radioactive sources according to different dosage. Tooth extraction was carried out at certain period before and after radiotherapy. Local X - ray and histopathological examination of jaws were performed regularly. Results Deep X-ray irradiation in guinea pigs showed that either the degree of jaw injury or the systemic reaction were much more severe than that of 60Co. After 1 month of irradiation, the osteoblasts were inhibited and the activity of osteoclasts was increased, resulting in bone resorption. After 2-4 months, The number decreased, nuclear pyknosis, empty lacuna increased, small periodontal ligament occlusion. A total of 7 cases (14.6%) had ORNJ in 3-5 months after radiotherapy. Conclusion The same dosage of deep X-ray damage to the jaw tissue is much more than 60Co; extraction of the jaw and other traumatic factors of the jaw have a role in inducing the occurrence of ORNJ
