目的探讨C反应蛋白(CRP)水平在重症及危重甲型H1N1流感和肺部细菌感染患者中是否有区别。方法对2009年9月23日-2010年1月20日,对多家医院收治的65例甲型H1N1流感(H1N1组)和37例肺部细菌感染患者(细菌感染组)的CRP水平、白细胞计数等进行比较。结果与细菌感染组相比,H1N1组CRP计数明显降低(102.5±14.9,27.3±3.2),差异有统计学意义(P<0.01),白细胞计数亦明显降低(16.1±2.1,6.7±0.9),差异有统计学意义(P<0.05);CRP曲线下面积为0.904(CI=0.799~1.010);CRP浓度对于细菌感染的鉴别值为43.5mg/L,其敏感性为86.7%,特异性为85.9%。结论 CRP水平可以帮助区别重症及危重甲型H1N1流感和肺部细菌感染患者。
Objective To investigate whether C-reactive protein (CRP) level is different between severe and critically ill patients with type A H1N1 influenza and pulmonary bacterial infections. Methods From September 23, 2009 to January 20, 2010, CRP levels in 65 cases of type A H1N1 influenza (H1N1 group) and 37 cases of pulmonary bacterial infection (bacterial infection group) in multiple hospitals were compared. The levels of leukocyte Count and other comparison. Results Compared with the bacterial infection group, the CRP counts in H1N1 group were significantly lower (102.5 ± 14.9 and 27.3 ± 3.2), the difference was statistically significant (P <0.01) and the leukocyte count was significantly lower (16.1 ± 2.1, 6.7 ± 0.9) (P <0.05). The area under CRP curve was 0.904 (CI = 0.799-1.010). The discriminant value of CRP for bacterial infection was 43.5 mg / L, with a sensitivity of 86.7% and a specificity of 85.9 %. Conclusion CRP levels can help distinguish severe and critically ill patients with type A H1N1 influenza and lung bacterial infections.
