目的:探讨彩色多普勒超声在多囊卵巢综合征患者中医证候分型中的应用价值。方法:选取符合PCOS诊断标准的患者198例进行研究,198例患者按照中医辨证分型标准分为肾虚肝郁50例,肾虚血瘀44例,脾虚痰湿74例,痰瘀互结30例,采用飞利浦IU-22彩色多普勒超声诊断仪检查卵巢形态和血流动力学参数。结果:脾虚痰湿和痰瘀互结组卵巢体积高于肾虚肝郁、肾虚血瘀组(P<0.05),脾虚痰湿组高于痰瘀互结组(P<0.05)。脾虚痰湿组、痰瘀互结组卵泡数大于肾虚肝郁组和肾虚血瘀组(P<0.05),脾虚痰湿组大于痰瘀互结组(P<0.05)。组间卵巢面积、髓质面积、面积比值和BMI比较,差异有统计学意义(P<0.05),脾虚痰湿组高于其余3组(P<0.05)。肾虚血瘀组和痰瘀互结组PI、RI高于其余两组(P<0.05),PSV、EDV低于其余两组(P<0.05),肾虚血瘀组与痰应互结组PI、RI、PSV和EDV差异有统计学意义(P<0.05)。结论:彩色多普勒超声检查在PCOS辨证分型中有重要应用价值。
Objective: To investigate the value of color Doppler ultrasound in the diagnosis of polycystic ovary syndrome patients with TCM syndromes. Methods: A total of 198 patients with PCOS diagnostic criteria were selected for the study. 198 patients were divided into 50 cases of kidney deficiency and liver depression, 44 cases of kidney deficiency and blood stasis, 74 cases of spleen deficiency and phlegm, 30 cases of phlegm and blood stasis, The morphological and hemodynamic parameters of ovaries were examined by Philips IU-22 color Doppler sonography. Results: The ovarian volume of phlegm - dampness and phlegm - blood stasis group was higher than that of kidney - deficiency and liver - qi stagnation group (P <0.05), and that of Spleen - phlegm - dampness group was higher than that of phlegm and blood stasis group (P <0.05). The numbers of follicles in phlegm-dampness-phlegm-dampness group and phlegm- stasis-phlegm-blood group were greater than that in kidney deficiency stagnation and blood stagnation group (P <0.05), phlegm and phlegm-dampness group (P <0.05). There were significant differences in ovarian area, medulla area, area ratio and BMI between the two groups (P <0.05), those in the spleen deficiency phlegm - dampness group (P <0.05). The levels of PI and RI in kidney deficiency and blood stasis group and phlegm and blood stasis group were higher than those in the other two groups (P <0.05), while PSV and EDV were lower than those in the other two groups (P <0.05) The differences of RI, PSV and EDV were statistically significant (P <0.05). Conclusion: Color Doppler ultrasound in PCOS syndrome differentiation has important application value.
