目的:采用经气管多普勒超声观察地氟醚对高血压病人血液动力学的影响.方法:全麻病人30例,高血压Ⅱ期15例(高血压组),美国麻醉学家学会(ASA)1级15例(对照组).芬太尼、依托咪酯、维库溴铵诱导后,插入多普勒气管导管,观察不同最小肺泡浓度(MAC)地氟醚对两组平均动脉压(MAP)、全身血管阻力(SVR)、心率(HR)、中心静脉压(CVP)、心脏指数(CI)的影响.结果:两组MAP、SVR随地氟醚MAC的升高呈剂量依赖性下降,HR、CVP呈剂量依赖性增加,0.5~2.0MAC时两组的CI均无显著变化.结论:地氟醚浓度<1.5MAC时对高血压病人的血液动力学影响较小,>2.0MAC时则应谨慎.“,”Objective: Our paper was designed to evaluate the effect of desflurane on hemodynamics in patients with hypertension throngh transtracheal dopper tube. Methods: For the 30 patients underwent general anesthesia, 15 patients with hypertension stage Ⅱ were assigned to group Ⅰ (hypertension group), and 15 patients with American society of Anesthesiologist(ASA) grade I were assigned to group Ⅱ (Control group). Intubation was performed with transtracheal dopper after induction by fentanyl, etomidate and vecuronium. The effects of desflurane on mean arterial pressure (MAP), systemic vascular resistance (SVR), heart rate (HR), central venous pressure (CVP), and cardiac index (CI) in both groups were measured at different minimal alveolar concentration (MAC). Results: MAP, SVR in both groups decreased dose-dependently with increase of MAC of desflurane, while HR, CVP increased dose-dependently. CI showed no significant change when desflurane was at 0.5-2 MAC. Conclusion: The effect of desflurane on hemodynamics in patients with hypertension was slight when its concentration was lower than 1.5 MAC, while much attention should be paid when desflurane concentration was over 2.0 MAC.
