目的通过对管状吻合器在肝门部胆管癌手术中胆肠吻合的应用研究,确定管状吻合器在肝门胆管空肠吻合的应用是切实可行的,优于传统的手工吻合。方法对于肝门胆管肿瘤局限于肝总管或分叉部(BismuthI/Ⅱ型)的病例,随机设立管状吻合器组(26例)和非吻合器组(24例),管状吻合器组:26例应用管状吻合器行胆管空肠Roux-en-Y吻合手术;非吻合器组:24例行常规手工胆肠吻合的病人,对二组手术病人的手术时间、术后并发症的临床数据行统计学的相关分析。结果应用管状吻合器行胆肠吻合是科学的方法,手术时间缩短,P<0.05;术后住院天数缩短,P<0.05;术后相关的并发症发生率降低,P<0.05。结论对于BismuthI/Ⅱ型肝门部胆管癌应用管状吻合器行肝门胆管空肠吻合术是可行的,较非吻合器组能缩短手术时间、减少术后并发症以及缩短住院时间。
Objective To evaluate the application of tubular stapler in choledochojejunostomy of hilar cholangiocarcinoma, and to determine the application of tubular stapler in jejunostomy of hilar cholangiocarcinoma, which is better than traditional manual anastomosis. Conclusions The application of tubular stapler in the treatment of Bismuth I / II hilar cholangiocarcinoma is feasible for the treatment of hilar cholangiojejunostomy, which can shorten the operation time, reduce the postoperative complications and shorten the hospitalization time.
