患者,男,21岁,因右膝包块14年、穿破流脓不愈2年,于85年11月19日入院。病员7岁时发现右膝内侧有一指头大小包块,活动,稍硬,疼痛不剧,膝关节活动正常。10多年来包块长大,不明显,入院前2年右膝撞伤后包块逐渐长大,右膝肿胀,轻微疼痛,于当地行包块切除。术后一月切口处发红,并在膝前又长多数软结节,逐长大穿破,溃烂,经久不愈入本院治疗。检查:右膝有多个散在的、大小不等高出皮肤的结节约蚕豆大小,部分顶部皮肤溃破形成窦口,周围皮肤呈紫黑色,界限清楚,压痛轻,中等硬度活动,已穿破的与皮肤紧密粘连,压窦口下结节,有褐黄色血性分泌物溢出,膝关节活动正常,浮髌(-),无明显肌萎缩。

Check: The right knee has a number of scattered knots, ranging in size from the skin to save the size of the beans, part of the top skin ulceration to form a sinus ostium, the surrounding skin is purple-black, the boundaries are clear, tenderness is light, the medium hardness activity, has been worn The close adhesion with the skin, sinus ostia, subcutaneous nodules, brownish bloody discharges overflowed, normal knee movements, impetuous (-), no significant muscle atrophy.
