目的 :研究实验室检测与超声 ,CT影像学在GRAVES眼病中的诊断价值 ,了解早期GRAVES病眼型的实验室特点和影象学改变。方法 :回顾性分析已经确诊为GRAVES眼病的 32例患者的实验室检测结果及眼眶超声、CT扫描等临床资料。结果 :32例中FT3FT4均在正常范围内 ,TRH兴奋实验中出现不能兴奋或弱反应的有 2 6例 ,符合率为 81.3% ;影像学特征为 :单眼或双眼多条眼外肌呈梭型肿胀增粗 ,眼眶B超诊断符合率为 87.5 %。CT扫描诊断符合率为 78.1%。B超和CT结合分析诊断符合率为 90 .6 %。结论 :GRAVES病早期甲状腺功能可正常 ,下丘脑 -垂体 -甲状腺功能异常 ,实验室检测与影像学对诊断GRAVES眼病都有较高的符合率 ,可作为GRAVES眼病诊断的重要依据。
Objective: To study the diagnostic value of laboratory testing, ultrasound and CT imaging in GRAVES eye diseases and to understand the laboratory features and imaging changes of early GRAVES eye diseases. Methods: The laboratory results of 32 patients with confirmed GRAVES eye disease were retrospectively analyzed. The clinical data including orbital ultrasound and CT scan were retrospectively analyzed. Results: Twenty-three patients had FT3FT4 in the normal range. Twenty-six patients with excitability or weak reaction in the TRH stimulation test were found, the coincidence rate was 81.3%. The imaging features were: Swelling thickening, orbital B-ultrasound diagnosis was 87.5%. The coincidence rate of CT scan diagnosis was 78.1%. B ultrasound and CT combined diagnostic accuracy of 90.6%. Conclusions: The early stage of GRAVES disease may have normal thyroid function, hypothalamus - pituitary - thyroid dysfunction, laboratory test and imaging for the diagnosis of GRAVES eye disease have a high coincidence rate, which can be used as an important basis for the diagnosis of GRAVES eye disease.
