目的:探讨抗突变型瓜氨酸波形蛋白(anti-MCV)抗体、抗RA33抗体和类风湿因子(RF)联合检测在类风湿关节炎(RA)中的临床应用价值。方法:对95例RA患者和50例其他自身免疫疾病患者及50名正常对照者检测抗MCV、抗RA33抗体和RF,观察各指标及3项联合检测对RA的诊断价值。结果:单指标检测抗MCV抗体对RA的敏感性最高;抗RA33抗体对RA的特异性最高。三项指标中任一指标阳性即作为阳性的敏感性最高(95.12%);而三指标均为阳性才视为阳性的敏感性略低于RF,但特异性可达99.00%。结论:抗MCV、抗RA33抗体、RF是RA较为特异的指标,3项指标联合检测可以把阳性患者的误诊率降到最低,有助于对RA的早期诊断、治疗和病情预后估计。
Objective: To investigate the clinical value of combined anti-MCV antibodies, anti-RA33 antibodies and rheumatoid factor (RF) in rheumatoid arthritis (RA). Methods: The anti-MCV, anti-RA33 antibody and RF were detected in 95 patients with RA and 50 patients with other autoimmune diseases and 50 normal controls. The diagnostic value of each index and 3 combined tests was evaluated. Results: Monoclonal antibodies against MCV were most sensitive to RA and anti-RA33 antibodies were the most specific for RA. The sensitivity of any one of the three indicators was highest (95.12%), while the sensitivity of the three indicators were positive only slightly lower than that of RF, but the specificity was 99.00%. Conclusion: Anti-MCV, anti-RA33 antibody and RF are the more specific indicators of RA. Combined detection of three indexes can minimize the misdiagnosis rate of positive patients and contribute to the early diagnosis, treatment and prognosis estimation of RA.
