Preliminary study of splenic artery ligation combined with selective peripheral vascular disarticulation fortreatment of hypersplenism with portal hypertension Purpose To observe the efficacy of splenic artery ligation combined with selective peripheral vascular disarticulation for treatment of hypersplenism with portal hypertension.Information and MethodsIn this study, 23 patients with portal hypertension underwent surgery treatment in Shengjing Hospital from February 2012 to March 2013. We treated 9 patients who met the inclusion criteria with the surgery of splenic artery ligation (SAL) and selective peripheral vascular disarticulation, and treated 14 patients who didnt met the inclusion criteria with splenectomy plus selective peripheral vascular disarticulation. Results All the 23 cases were operated successfully without death. The SAL group cut down the portal venous pressure and there is no statistics difference between the two groups.There were rises in both WBC and PLT counts, but the group went spenectomy rose more (P<0.05). 3 patients in the SAL group got complications, one was PVT and the other two were infarction; 9 patients in the splenectomy group got the complication of PVT, higher than the SAL group. ConclusionTreating the patients who met the inclusion criteria with splenic artery ligation and selective peripheral vascular disarticulation can do both correct the hypersplenism, but also reduce the incidence of postoperative complications.
