Background Early in 1968, USA first created the anticoagulation clinic, then many of the worlds developed countries and regions, and Hongkong and Taiwan of China successively established anticoagulation clinic.Practice has proved that the anticoagulation clinic, in which pharmacist has been in charging of drug therapy management and the other related work, has the vital role and the significance to guarantee the treatment effect and drug safety in patients who need anticoagulation therapy.Since March 2013, Beijing Tiantan Hospital has set up Beijings first physician-pharmacist collaborative anticoagulation clinic, pharmacist has takes the responsibility of comprehensive medication management, medication education, providing consultation and long-term follow-up, etc.Objective To introduce the practice and experience on pharmacist participating physician-pharmacist collaborative anticoagulation clinic.Methods The following aspects were summarized: pharmacists practice situation in this anticoagulation clinic, pharmacist and physicians cooperation mode, the procedure, medication management, medication education, long-term follow-up, consultation pharmacist has provided for patients and the prominent cases pharmacist ensured the medication safety of patients, and the other work.The working experience of pharmacist was also summarized.Results and conclusion For more than one year since the cardiovascular anticoagulation clinic of Beijing Tiantan Hospital was set up, good results have been achieved, and this clinic has got the recognition and praise from all the patients who visited it, on the whole, this anticoagulation clinic has formed a trend of consistent growing in number of patients and cumulative person-time with an already set-up patients population base.It is of great significance and clinical importance to set up anticoagulation clinic in which pharmacist participates in, and this mode is worthy of clinical expansion.
