目的 探讨血清瘦素与高血压、体重指数 (BMI)、能量代谢间的关系。方法 测定 64例男性原发性高血压患者及 5 4例男性正常血压者的血清瘦素浓度、收缩压 (SBP)、舒张压 (DBP)、BMI、血糖 (Glu)、甘油三酯 (TG)、总胆固醇 (TC)、高密度脂蛋白 (HDL)、低密度脂蛋白 (LDL)等指标。血清瘦素用放射免疫法测定。分析瘦素与其它各项参数的相关性。结果 ( 1)两组 ( 118例 )的血清瘦素浓度 ( 3 3± 1 9) μg/L与BMI、SBP、DBP、Glu、TG、TC、LDL等呈显著正相关 (P <0 0 5~P <0 0 0 0 1) ;( 2 )高血压组血清瘦素浓度 ( 3 8± 1 9) μg/L显著高于正常组 ( 2 8± 1 3 ) μg/L(P <0 0 1) ,两组血清瘦素浓度与BMI差异有非常显著意义 (P均 <0 0 0 1)。正常组血清瘦素浓度与SBP显著相关 (P <0 0 5 )。但经协方差分析校正BMI和年龄后 ,两组间血清瘦素浓度差异无显著意义 ;( 3 )在剔除超重、体重过轻、血糖、血脂异常者后 ,高血压组 ( 3 9例 )与正常组 ( 4 0例 )的血清瘦素浓度分别为 [( 3 2± 1 6) μg/L、( 2 8± 1 2 ) μg/L],两者差异无显著意义 ,但仍与BMI显著相关 ,与血压值无显著相关 ;结论 血清瘦素浓度与血压无直接相关关系 ,而与人体肥胖程度有密切关系 ,与能量代谢也可能有关。
Objective To investigate the relationship between serum leptin and hypertension, body mass index (BMI) and energy metabolism. Methods Serum leptin concentration, systolic blood pressure (SBP), diastolic blood pressure (DBP), BMI, Glu, triglyceride (TG) were measured in 64 male patients with essential hypertension and 54 male patients with normal blood pressure. , Total cholesterol (TC), high-density lipoprotein (HDL), low-density lipoprotein (LDL) and other indicators. The level of serum leptin in normal group was significantly correlated with SBP (P <0.05). However, after adjusted for BMI and age by covariance analysis, there was no significant difference in serum leptin concentration between the two groups. Conclusion There is no direct correlation between serum leptin level and blood pressure, but it is closely related to the degree of obesity in human body and may be related to energy metabolism.
