目的:观察Ⅱ型呼吸衰竭时患者甲状腺素(T4)、抗利尿激素(ADH)代谢的变化特点及低T3综合征,抗利尿激素分泌失调综合征的临床意义。方法:对27例Ⅱ型呼吸衰竭患者进行电解质、甲状腺素、皮质醇和24小时尿钠检测,结果:12例低钠血症的患者中5例伴有高渗尿;27例患者中24例出现三碘甲状腺原氨酸(T3)降低。结论:Ⅱ型呼吸衰竭可致抗利尿激素分泌失调综合征及低T3综合征;前者使病情复杂化,后者与预后有关。
Objective: To observe the changes of thyroid hormone (T4) and antidiuretic hormone (ADH) metabolism in patients with type Ⅱ respiratory failure and the clinical significance of low T3 syndrome and antidiuretic hormone secretion syndrome. Methods: Electrolytes, thyroxine, cortisol and 24-hour urinary sodium were tested in 27 patients with type Ⅱ respiratory failure. Results: 5 of 12 patients with hyponatremia were accompanied by hyperuria and 24 of 27 patients Triiodothyronine (T3) decreased. CONCLUSION: Type II respiratory failure can cause anti-diuretic hormone syndrome and hypo-T3 syndrome. The former complicates the condition and the latter relates to the prognosis.
