目的在原发性高血压人群中,探讨体质指数(BMI)和血脂水平与瘦素受体(LEPR)基因A223G突变关系。方法选取上海市黄浦区原发性高血压人群进行问卷调查、体检、生化指标和A223G突变的检测。结果902例原发性高血压患者中,体重正常组(BMI<24kg/m2)400例,超重肥胖组(BMI≥24kg/m2)502例,男性超重肥胖占59.3%,高于女性50.4%(P<0.01)。A223G基因型多态性分布:男性AA、AG、GG分别为2.8%,19.8%,77.4%;女性分别为4.9%,23.4%,71.7%。调整相关影响因素后,A223G多态性G等位基因与甘油三酯(TG)、甘油三酯/高密度脂蛋白比值(TG/HDL)水平升高以及HDL水平降低显著相关(P<0.05)。两因素间交互作用检验结果显示,A223G与超重及肥胖对原发性高血压患者的HDL水平降低存在交互作用关系,调整混杂因素后,交互作用系数95%CI=0.0~0.1(P=0.042)。结论LEPR基因A223G突变和血脂水平相关,并可调节超重及肥胖与HDL的关系。
Objective To investigate the relationship between body mass index (BMI) and serum lipids and A223G mutation of leptin receptor (LEPR) gene in essential hypertension. Methods The subjects with essential hypertension in Huangpu District of Shanghai were investigated by questionnaires, physical examination, biochemical indexes and A223G mutation. Results Of the 902 patients with essential hypertension, there were 400 cases of normal weight group (BMI <24kg / m2) and 502 cases of overweight and obesity group (BMI≥24kg / m2). The prevalence of overweight and obesity was 59.3% in males and 50.4% P <0.01). The genotype distribution of A223G genotypes was AA, AG and GG were 2.8%, 19.8% and 77.4% in male and 4.9%, 23.4% and 71.7% in female respectively. After adjusting for the related factors, the G allele of A223G polymorphism was significantly associated with the increase of triglyceride (TG), triglyceride / high density lipoprotein ratio (TG / HDL) level and the decrease of HDL level (P <0.05) . The interaction between A223G and the patients with overweight and obesity showed that there was an interaction between A223G and HDL levels in patients with essential hypertension. After adjustment for confounding factors, the interaction coefficient was 95% CI = 0.0-0.1 (P = 0.042) . Conclusions A223G mutation of LEPR gene is associated with blood lipid level and regulates the relationship between overweight and obesity and HDL.
