美国医疗照顾计划加上其他私人医疗保险,使得美国大部分65岁及以上的老年人口都能得到基本的医疗保障,但人口老龄化、经济危机等现实困境促使美国政府不得不采取多种措施控制医疗照顾计划的支出成本。虽然利用市场竞争来增加效率、降低医疗照顾计划费用是一种有效方法,但《患者保护与平价医疗法案》(PPACA)从另一个角度证明,政府在医疗保险费用控制方面可以起到很重要的作用。本文立足于美国PPACA出台以后,医疗照顾计划四个保障领域的控费实践及法案的其他条款对医疗照顾计划控费的影响,通过PPACA法案出台前后的数据比较,发现近年来尽管医疗照顾计划费用总支出还在逐年增长,但支出增长率开始趋于平缓。这一政策对我国医改有着重要的借鉴作用。
