目的:对漏斗胸患者术后致左胸壁导联R波降低ST抬高的原因进行探讨。方法:选取我院2011年6月治疗的1例漏斗胸术后致左胸壁导联R波降低ST抬高的患者,通过对他的体检报告、胸片、心电图、心脏超声、临床诊断等资料进行对比与分析。结果:患者手术前检查ST—T无异常,置入金属支撑杆进行治疗。在术后3年复查时发现患者左胸壁导联R波降低ST抬高,与置入金属支撑杆有关,金属支撑杆损伤患者心室壁,造成心肌损害。结论:心电图改变机制尚未明确,猜测与置入患者胸腔内的金属支撑杆有关,造成导电不良,从而导致R波振幅降低,而ST段抬高主要是金属支撑杆损害患者心肌造成的。
OBJECTIVE: To investigate the causes of postoperative left-chest lead R wave reduction of ST elevation in patients with funnel chest. Methods: One patient with funnel thoracotomy treated in our hospital in June 2011 was selected to reduce RST in patients with R wave of left chest wall. Through his physical examination reports, chest radiography, electrocardiogram, echocardiography, clinical diagnosis and other data For comparison and analysis. Results: The patients had no abnormalities of ST-T before operation, and placed metal support rods for treatment. Three years after the procedure, we found that the R wave in the left chest wall led to the decrease of ST elevation, which was related to the placement of the metal support rod. The metal support rod damaged the ventricular wall and caused myocardial damage. CONCLUSION: The mechanism of electrocardiogram change is not yet clear. It is speculated that it is related to the metal support rod inserted into the chest cavity of the patient, resulting in poor conduction, leading to the decrease of R wave amplitude. ST segment elevation is mainly caused by the metal support rod damaging the myocardium.
